Insurance Waiver and Release Form
Dear Parent:
We know that it is your wish as well as ours that every precaution be taken to protect your child from injury.
We do our utmost to promote this by proper training, proper equipment usage, through direct supervision of all activities, and encouraging good safety habits.
We are confident that with adherence to the rules, policies, and procedures of the coaches and/or instructors the athletes in our program will have a positive, beneficial, and safe training experience. Despite our efforts, accidents do happen occasionally in athletics as elsewhere. The Batter’s Box, Staff, Trainers, Coaches and its’ affiliates assumes no legal liability for medical or hospital expenses resulting from athletic injuries incurred in training or the use of our facilities, but we certainly want to do our part to insure the best possible protection for your child. Because of this it is important to us to have your written permission and consent for their participation as well as your acknowledgement that you have the proper health insurance to cover your child.
In order for our athletes to be insured as well as possible, we require each participant provide us with a waiver form which explains that the athlete is adequately insured with your own personal family insurance. In no case will The Batter’s Box, Staff, Trainers, Coaches and its’ affiliates of use be responsible for the cost of prescriptions, special medications, and the like.
This is to acknowledge that my child is adequately covered by our own personal insurance against injuries sustained in training athletes. I understand that I am releasing The Batter’s Box, Staff, Trainers, Coaches and its’ affiliates from all liability in the event of an accident.
I hereby give The Batter’s Box, Staff, Trainers, Coaches and its’ affiliates permission to take photographs or use video and sound in which he/she (participant) may be involved with others without compensation to me or my child. These photographs may be used by The Batter’s Box, Staff, Trainers, Coaches and its’ affiliates for promotional and informational purposes in print, or on the The Batter’s Box, Staff, Trainers, Coaches and its’ affiliates website and in other media

Notice of Sufficient Physical Ability/ Warning and Notification of Risk
I hereby guarantee that my child is in adequate physical and mental health to participate in athletic training exercises. I authorize The Batter’s Box, Staff, Trainers, Coaches and its’ affiliates to train and condition my child for the period agreed upon by both parties. Playing, practicing, or participating in any sport can be a dangerous activity involving risks of injury. The dangers and risks are sometimes apparent and other times they are unforeseeable. The Batter’s Box, Staff, Trainers, Coaches and its’ affiliates assumes no legal liability or responsibility for any level of foreseeable or unforeseeable risk of injury.
I/We am the parent(s)/legal guardian(s) of the above named child. I/We have read the Warning and Notification of Risk and understand the risks of our child participating in training activities.
